What does it take to be the best nurse or nurse assistant in long-term care?
The Center for Excellence in Aging Services and Long-Term Care wants to know
The Center for Excellence in Aging Services and Long-Term Care is seeking nominations for healthcare providers at the CNA, LVN, RN, and APRN levels who have provided excellent person-centered care to patients in a long-term care setting. This can be at a nursing home or skilled nursing facility.
We are accepting nominations that fit the following criteria:
- Work in long-term care setting in Texas
- Must be at least a year in position
- Demonstrates past or present person-centered care
Nomination process: Submit 250 words or less on why you think the nominee is deserving of this award. Self-nominations are acceptable. Please use the online submission form found at the bottom of this page.
Selection process: A committee will review each entry and select a winner under the categories of nurse, nurse assistant, and nurse administrator. The submission deadline is Tuesday, June 30, 2020 at noon.
Winners: Winners will be featured on the Center for Excellence in Aging Services and Long-Term Care’s (CEASLTC) website and social media pages. Each winner will also receive a certificate and be honored at an awards ceremony at the annual Geriatrics Symposium, sponsored by Texas Health and Human Services and co-sponsored by the CEASLTC.
We want to recognize and share with others the types of work that nurses do.
