What does it take to be the best nurse or nurse assistant in long-term care?

The Center for Excellence in Aging Services and Long Term Care wants to know.

The Center for Excellence in Aging Services and Long Term Care is seeking nominations for healthcare providers at the CNA, LVN, RN, and APRN levels who have provided excellent person-centered care to patients in a long-term care setting. This can be at a nursing home or skilled nursing facility.

We are accepting nominations that fit the following criteria:

  • Work in long-term care setting in Texas
  • Must be at least a year in position
  • Demonstrates past or present person-centered care

Nomination process: Submit 250 words or less on why you think the nominee is deserving of this award. Self-nominations are acceptable. Please use the online submission form found at the bottom of this page.

Selection process: A committee will review each entry and select a winner under the categories of nurse, nurse assistant, and nurse administrator. The submission deadline is Friday, May 24, 2019 at noon.

Winners: Winners will be featured on the Center for Excellence in Aging Services and Long Term Care’s (CEASLTC) website and social media pages. Each winner will also receive a certificate and be honored at an awards ceremony at the annual Geriatrics Symposium, co-sponsored by Texas Health and Human Services and the CEASLTC.

We want to recognize and share with others the types of work that nurses do.
